What Is a Mastopexy or Breast Lift?
A mastopexy is a surgical procedure that lifts sagging, slack and deflated breasts. The main aim of the surgery is to raise the breasts, areolas and nipples and to reshape the breast tissue, achieving harmony of shape, size and position. It is also known as a breast lift, breast pexy or breast uplift.

Types of Mastopexy
- Mastopexy Without Silicone Implants: ideal for women with enough breast gland who do not wish to increase breast size. It lifts the sagging breast and reshapes the existing tissue for a firm, rounder and natural result. It can be combined with autologous fat injections to add a little volume and enhance the cleavage.
- Mastopexy With Silicone Implants: ideal for women with sagging breasts that have little volume and look deflated. The surgery lifts the breasts and, at the same time, increases their volume by placing silicone implants.
- Mastopexy With Breast Reduction: reduces overly large breasts (gigantomastia) and corrects ptosis. Mainly indicated for women with large breasts who experience problems such as back, neck and shoulder pain, skin irritation and discomfort during some daily routines.
- Mastopexy With Asymmetry Correction: correcting breast asymmetry may call for techniques as simple as using implants of different sizes, or for more complex ones such as various types of mastopexy, breast tissue remodeling or reducing the size of one breast. Every case is different and each breast requires its own tailored treatment. It is indicated for women with one or both breasts sagging and with noticeable differences that cause discomfort and undermine their self-confidence.
- Mastopexy After Implant Removal or Replacement: removing or replacing implants may become necessary after several years. In both cases the plastic surgeon has to calculate the exact measurements to lift the breast and adapt it to its new proportions.
How a Mastopexy Is Performed
Before starting the mastopexy, the plastic surgeon marks the lines where the incisions will lie – around the areola, a vertical line from the areola to the inframammary fold and a horizontal line along the fold. Once the patient is under anesthesia, the excess skin is removed and the breast gland is reshaped into a higher position. If the patient has little breast tissue or has lost a great deal of volume, silicone implants and/or fat grafting can be added. The incisions are closed with internal, absorbable sutures for a discreet scar, with no stitches to remove.
Scars After a Mastopexy
The type of scar depends on several factors: the degree of ptosis, the amount of excess skin, the amount of fatty and glandular tissue and whether or not the patient wants silicone implants. The most common scars are:
- Periareolar scar: a line around the areola, indicated for mild ptosis.
- Vertical scar: a scar around the areola plus a vertical line running from the areola to the inframammary fold. Indicated for moderate ptosis.
- Inverted T scar: the most extensive of the three, indicated for more severe degrees of ptosis. A line around the areola, followed by a vertical line from the areola to the fold and a horizontal line along the lower part of the breast.
Recovery and Postoperative Care
Recovery from a mastopexy is not very painful, but it is important that you follow every medical recommendation for a smoother postoperative period. No drains are needed. You will wear a bra for a month, day and night. The stitches are internal and absorbable, so there are none to remove.
After a mastopexy you should expect to be off work for 7 to 10 days, depending on the kind of job you do. After two weeks you can go back to most everyday activities, and after a month you can resume strenuous effort.
Price of a Mastopexy in Madrid
The price of a mastopexy in Madrid, Spain, depends on several factors: the experience of the plastic surgeon performing the surgery, the type of mastopexy and whether silicone implants and autologous fat transfer are used or not. The exact price is confirmed after a diagnostic consultation with the plastic surgeon.
